29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices were not singularly identified; were not the only devices used for controlling energy; were used for other purposes; and did not meet the requirements of subparagraphs (A) through (D): On or about: November 20, 2023 Location: 615 Ferry Street, Newark, NJ 07105 a) The employer did not ensure that specific lockout devices were singularly identified when using lock to lock out pneumatic pressure for 121-M-6 machine. All employees used the same lock to perform LOTO for pneumatics, which was located at the machine.
