29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Facility, Brickyard Road, Clairton, PA: On or about November 11, 2023, an employee suffered an amputation when his fingers came into contact with the unguarded point of operation on the Riken Seiki Co hydraulic cylinder on the brick splitter that is used to break refractory bricks into smaller pieces. TYK America, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1564425, citation number 1, item number 3a, instance b and was affirmed as a final order on 5/20/22, with respect to a workplace located at 301 Brickyard Road, Clairton, PA 15205.
