29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system. On or about November 14, 2023, employees were not protected from falling from a bridge after guardrails were removed. On or about November 14, 2023, an employee fell approximately 10-12 feet from a bridge and was hospitalized.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer did not report an employee hospitalization within 24 hours of being aware of the hospitalization. On or about November 14, 2023, an employee was hospitalized after falling from a bridge into a creek.
