Instances included but were not limited to: i. On or about November 17, 2023, approximately two employees repairing and/or servicing overhead dock door #5 while using one lock. ii. On or about January 8, 2024, approximately two employees repairing and/or servicing overhead dock door #3 while using one lock. iii. On or about January 16, 2024, approximately two employees repairing and/or servicing overhead dock door #11 while using one lock.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. Employees engaged in battery maintenance activities at United States Cold Storage located at 4302 South 30th Street Omaha, Nebraska were exposed to chemical contact and burn hazards in that the employer did not ensure the pathway between the battery maintenance area (charging and filling) and the emergency eyewash and shower remained unobstructed.
