29 CFR  1926.451(a)(1):Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it:  a) On or about 11/20/2023 and at times prior, at the jobsite located at 80 Rogers St, Clearwater, Florida 33756, employees were exposed to fall hazards of approximately 10 feet 3 inches, in that, the strength and integrity of the scaffold platform (styrofoam) was not adequate to support employees safely.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about 11/20/2023 and at times prior, at the jobsite located at 80 Rogers St, Clearwater, Florida 33756, employees were exposed to fall hazards of approximately 10 feet 3 inches on the South side of the building, in that the scaffold ladder rails did not extend 3 feet above the landing surface to which the ladder was used to gain access.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  a) On or about 11/20/2023 and at times prior, at the jobsite located at 80 Rogers St, Clearwater, Florida 33756, employees were exposed to fall hazards of approximately 10 feet, in that, fall protection was not used while on a supported scaffold.  The Wilson Kehoe Miller Corporation was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1470683, citation number 1, item number 1 and was affirmed as a final order on 05-27-2020, with respect to a workplace located at 464 Paula Drive, Dunedin, Florida 34698.
