29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  (a)  On or about November 28, 2023, the hourly men's change room was not clean, orderly or in a sanitary condition, in that there was a thick layer of black dust (potentially coke or coal dust) on top of the rows of lockers and on the floor.  Dirty work boots were stored on top of lockers.  Empty shampoo bottles, soap containers, scraps of paper, wet towels, and discarded tissues littered the floor.  Empty hangers were lying all over the floor and on benches.  The showers were dirty, moldy, and in disrepair.  (b)  On or about November 28, 2023. the hourly women's, salaried women's and salaried men's locker rooms were observed to have empty hangers on the floor, and dirty work boots stored on top of lockers, on the floor, and on benches.  Hard hats and other PPE were kept on top of lockers. The showers were dirty and moldy.   (c)  On or about November 28, 2023, the maintenance locker room was observed to have a dirty and a clean side, but there were still hangers strewn about the floor, boots and hard hats on top of the lockers and many space heaters throughout the locker room.  Black dust (potentially coke or coal dust) was tracked through the dirty side.  The showers were dirty and moldy.

29 CFR 1910.141(d)(3)(i):  Whenever showers are required by a particular standard, the showers shall be provided in accordance with paragraphs (d)(3)(ii) through (v) of this section.  (a)  On or about November 28, 2023, the employer did not ensure that body soap or other appropriate cleansing agents convenient to the showers were provided in appropriate quantities, as specified in paragraph (d)(2)(iii) of 29 CFR 1910.141.  (b)  On or about November 28, 2023, the employer did not ensure that showers were provided with enough hot water for all employees who were required to shower during a shift to have a hot or warm shower.

29 CFR 1910.141(e): Change rooms. Whenever employees are required by a particular standard to wear protective clothing because of the possibility of contamination with toxic materials, change rooms equipped with storage facilities for street clothes and separate storage facilities for the protective clothing shall be provided.   (a) On or about November 28, 2023, the employer did not ensure that the lockers provided in the change rooms were large enough to hold employees' street clothing, coats, and shoes on the clean side and personal protective equipment, including metatarsal safety shoes, hardhats, safety glasses, and work coats, on the dirty side.  (b)  On or about November 28, 2023, the employer did not ensure that employees did not store shower shoes and street shoes alongside contaminated safety shoes and hard hats on top of the rows of lockers in the change rooms.

29 CFR 1910.157(c)(1):  The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  (a)  On or about November 28, 2023, a portable fire extinguisher in the maintenance locker room was not properly mounted and located, as it was observed sitting on the floor behind a door.
