29 CFR 1910.304(b)(3)(ii)(A):  All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that were not part of the permanent wiring of the building or structure and that were in use by personnel did not have ground-fault circuit-interrupter protection for personnel.  On or about November 30, 2023, an electrical receptacle near a water source in the employee's breakroom did not have a ground-fault circuit interrupter, this condition exposed employees to electrical shock hazards.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.  On or about November 30,2023, in the RG Ray department, employees working in warehouse B were exposed to electrical shock hazards when the outer/inner insulation of the extension cord was severely damaged exposing the live copper conductors.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about November 30, 2023, employees in warehouse C working near dock 41 were exposed to electrical shock hazards when the employer did not ensure the strain relief provided prevented pulling of the electrical wires.
