29 CFR  1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   a) On or about 11/30/2023, on the site located at the corner of Bear and Buckley Road, Liverpool, NY: Employees engaged in truss work were not utilizing any fall protection. Employees were exposed to fall of 8 feet.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a)  On or about 11/30/2023, on the site located at the corner of Bear and Buckley Road, Liverpool, NY: Employees were using a ladder for roof access that was not extended 3 feet above the upper landing surface. Employees were exposed to a fall distance of approximately 8 feet.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step.  a)  On or about 11/30/2023, on the site located at the corner of Bear and Buckley Road, Liverpool, NY: Employees were using the top step of a stepladder to access the trusses . Employees were exposed to a fall distance of 8 feet.
