29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Main Shop (Ironworker - Punch Area): On or about November 30, 2023, and at times prior, the employer exposed employees to amputation hazards in that point of operation side guarding was not provided to employees performing metal punch operations for production on the Piranha Ironworker (Serial No.: P140 3088). b) Main Shop (Angle Roller Area): On or about November 30, 2023, and at times prior, the employer exposed employees to amputation hazards in that point of operation guarding was not provided to employees performing metal rolling operations for production on the COMAC Universal Bending Machine (Model No.: 305 HV Serial No.: 3051129). c) Main Shop (Metal Lathe Area): On or about November 30, 2023, and at times prior, the employer exposed employees to amputation hazards in that a chuck guard was not provided to employees performing metal boring operations for production on the Standard Modern Lathe (Serial No.: 6600).
