29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded or guards were not designed and constructed to prevent employees from having any part of their body in the danger zone during operating cycle(s): (a) PPD (USA) Inc. at 1810 Northeast 3rd Street, Madison, SD 57042: On or about and at times prior to December 4, 2023, the employer did not ensure that the point of operation on the Numatics Bearing Press (Model: 153SA431K000030) was adequately guarded to prevent employees from having any part of their body in the danger zone during machine operations. Two-handed operation safety devices were installed in a manner that allowed for devices to be easily defeated. This hazard exposed employees to crushing and amputation hazards.
