Records of inspections and maintenance were not kept in accordance with paragraph (e) of this section. a) Production: On or about 11/30/2023, the employer did not visually inspect and check that the pullout device(s) on the Press Rite press #0006 were affixed properly to the operator's hands. The straps of the pullout device(s) were loosely fitted and not properly secured.
