29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) Production Department; Grinder 1 Platform: On or about November 28, 2023, and at times prior, the employer exposed employees to slip and fall hazards; in that, the employer did not ensure the walking-working surface of the platform for Grinder 1 was maintained clear of a collection of plastic pellets and other small, recycled materials.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Production Department, Grinder Area: On November 28, 2023, and at times prior, the employer exposed employees to falls of approximately 7 feet to ground level; in that, the employer did not provide employees operating "Grinder 1" with fall protection as they stood on an elevated platform to shovel feed material into the machine.
