On or about December 6, 2023, and at time prior to, the employer exposed employees to amputation hazards in that employees were required to change blades on the trim saw without utilizing energy control procedures to secure all hazardous energy sources.

On or about December 6, 2023, and at times prior, the employer failed to provide energy control devices such as, but not limited to locks, tags, blocks, pins, wedges, or fasteners as required by the standard to properly protect against hazards associated with the release of stored energy, such as electrical, when changing the blades on the trim saw.
