29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about and at times prior to December 7, 2023, at the previously addressed inspection site, employees using pneumatic framing nailers when installing shingles and trim for the roof of a residential building did not use eye protection (safety glasses) exposing employees to eye hazards from flying particles and objects.  Melchor Windows and Siding LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1371067, citation number 1, item number 2 and was affirmed as a final order on February 26, 2019, with respect to a workplace located at 850 East Broad Way, Madison, WI 53716.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) On or about and at times prior to December 7, 2023, at the previously addressed inspection site, employees installing shingles for a residential building at heights 6 feet or more above lower levels were exposed to falls.  Melchor Windows and Siding LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1624204, citation number 1, item number 1 and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 3702 Welcome Dr, Middleton, WI 53713.   Melchor Windows and Siding LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1614895, citation number 1, item number 1 and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 1501 Wright St, Madison, WI 53704.
