29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Construction Site, 5273 Inland Ave, 273 Inland Ave, Firestone CO, 80504: On or about December 06, 2023, employees were exposed to fall hazards of approximately 12 feet to the ground below while installing asphalt shingles on the first story of a residential roof. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work. b) Construction Site, 5273 Inland Ave, 273 Inland Ave, Firestone CO, 80504: On or about December 06, 2023, employees were exposed to fall hazards of approximately 20 feet to the ground below while installing asphalt shingles on the second story of a residential roof. The ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.
