29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm: On or about December 5, 2024, a 2-tier (industrial steel) storage rack located in the produce prep room was not anchored to the floor. Employees who retrieved produce from the storage rack, as well as other employees working in the immediate area, were potentially exposed to struck-by and/or crushing hazards if the storage racks were to fall or tipped over.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made: On or about December 5, 2024, a Dynamo 12 Gallon Wet/Dry Vacuum (Serial Number: ARA8778876) used by employees when cleaning out an onion bin(s) that was located in the produce area of the store, was plugged into an extension cord that was missing the ground pin. Employees and others working in the immediate vicinity were potential exposed to electrical hazards if there were to be a short in the electrical equipment, circuit breakers were too short/fail, or water were to be accidently introduced into the area. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1431068, citation number 3, item number 2, and was affirmed as a final order on May 6, 2020, with respect to a workplace located at Quantico, VA.
