29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Midway Products Group, Inc. doing business as, Lakepark, Industries Inc., located at 40 Seminary Street, Greenwich, Ohio: On or about December 1, 2023, the employer failed to ensure employees were adequatly protected from crushed-by hazards while performing activities such as, but not limited to, clearing scrap and making adjustments to the to the shaker trays/chutes on the oscillating actuator on Press #4 in the Press Department. The employer failed to ensure that the energy control procedures were utilized to control the presses' energy sources prior to employees performing these task.
