29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section: On or about December 13, 2023, employees engaged in maintenance of equipment at 22801 Fairview Road, Gretna, Nebraska were exposed to electrical shock, amputations, and caught in between hazards. The employer failed to establish lockout/tag-out procedures for each piece of equipment to be locked out to include reenergizing.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about December 13, 2023, employees engaged in maintenance of equipment at 22801 Fairview Road, Gretna, Nebraska were exposed to chemical hazards. The employer failed to ensure all employees received training who work with chemicals, such as, but not limited to: brake cleaner and Torch It.

The employer failed to provide lockout tagout procedures to outside contractors performing maintenance on production equipment.
