29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: a) Oldcastle BuildingEnvelope, Inc. dba Graham Architectural Products, 1201 Eden Road, York, PA 17402 - On or about December 13, 2023, employees were overexposed to carbon monoxide generated by a faulty exhaust system on the Combilift C6000 Multidirectional propane forklift (S/N: 31154). Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle: a) Oldcastle BuildingEnvelope, Inc. dba Graham Architectural Products, 1201 Eden Road, York, PA 17402 - On or about December 13, 2023, employees were operating, and working in proximity to, a Combilift C6000 Multidirectional propane forklift (S/N: 31154) and were exposed to carbon monoxide from a faulty exhaust system on the forklift. A prior maintenance visit and inspection of the forklift indicated continued repair needs for the forklift, however the forklift was place back into service for use prior to these repairs being made. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide gas listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: a) Oldcastle BuildingEnvelope, Inc. dba Graham Architectural Products, 1201 Eden Road, York, PA 17402 - On or about December 13, 2023, employees were overexposed to carbon monoxide at approximately 5.6 to 8.9 times the PEL of 50 ppm due to a faulty exhaust system on the Combilift C6000 Multidirectional propane forklift (S/N: 31154). Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
