29 CFR 1910.134(g)(1)(ii): Corrective glasses, goggles or other personal protective equipment were worn in a manner that interfered with the seal of the respirator facepiece:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD) On or about December 13, 2023, and times prior thereto, in the yard the employer did not ensure employees using tight fitting respirators had a seal to prevent contaminants from reaching the breathing zone. An employee wore a respirator while wearing a cloth balaclava masks between their face and the respirator which impeded the face seal while conducting hot work.

29 CFR 1910.334(a)(2)(ii):Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket). On or about December 13, 2023, and times prior thereto, the employer failed to ensure the grinder cord was not frayed or damaged. The exposed conductors created and electrical shock hazard to the employees.
