29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Reyes Brothers Construction LLC, on or about October 18, 2023 at the address of 2310 Jaycox Road, Avon, OH : The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a two-story, single family residential structure. Employees operating in this condition were exposed to fall injuries from heights of 10-12 feet to the ground below. Reyes Brothers Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1648528, citation number 2, item number 2, and was affirmed as a final order on 5/2/23, with respect to a workplace located at 2078 Pine Grove Lane Columbus, Ohio, 43232.

29 CFR 1926.1053(b)(1):The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support: On or about October 18, 2023, at the location of a worksite located at 2310 Jaycox Road, Avon, OH: employees were exposed to a fall hazard of approximately 11 feet while performing roofing activities on a single family home . Employees were using an extension ladder to access the roof and the employer did not ensure they extended at least 3 feet above the upper landing surface. Reyes Brothers Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1648528, citation number 1, item number 1, and was affirmed as a final order on 5/2/23, with respect to a workplace located at 2078 Pine Grove Lane Columbus, Ohio, 43232.
