29 CFR 1926.501(b)(4):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 1681 Sherman Ave., Pennsauken, NJ: Employees were working near skylights and openings in the roof that were not protected against fall hazards. One employee backed into one of the openings and fell approximately twenty feet to the floor below. Observed on or about 12/14/2023 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(h)(1):The employer did not did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section: a) 1681 Sherman Ave., Pennsauken, NJ: Employees were observed working on a flat roof without a safety monitor. Ten employees were working behind warning lines and exposed to fall hazards of approximately twenty feet to the ground below. Observed on or about 12/14/2023 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
