29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On a 6:12 pitch roof, at a worksite located at 317 Key West Drive, Jacksonville, Florida 32225: On or about December 14, 2023, the employer exposed six employees to fall hazards, in that, fall protection were not utilized by the employees while tearing off an old roof system. The employees were exposed to the hazard of falls ranging from 8 feet to 24-foot to the ground.

Right side of home located at 317 Key West Drive, Jacksonville, FL 32225: On or about December 14, 2023, the employer exposed six employees to fall hazards in that, the ladder used to gain access to the roof, the side rails did not extend three feet above the roof, exposing employees to an 8 feet fall hazard.
