At the northwest of the residential building in 5909 NW 43rd Ave., Gainesville, Florida: On or about February 6, 2024, the employer exposed employees to fall hazards, in that employees removing old roof shingles were not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.

At the west area of the residential roof, located at 5909 NW 43rd Ave., in Gainesville, Florida: On or about February 6, 2024, the employer exposed employees to fall hazards, in that fall protection was not utilized by the employees while performing roofing work.  b.  At the northeast area of the residential roof, located at 5909 NW 43rd Ave., in Gainesville, Florida: On or about February 6, 2024, the employer exposed employees to fall hazards, in that fall protection was not utilized by the employees while performing roofing work.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a.  At 5909 NW 43rd Ave., Gainesville, Florida, on or about February 6, 2024, a roofing employee who worked at elevations on steep pitch roofs had not been trained by a competent person to recognize fall hazards and the use of personal fall arrest systems, guardrail system, safety net systems, warning line systems, and other protections.

At 5909 NW 43rd Ave., Gainesville, Florida, on or about February 20, 2024, the employer failed to report a work-related in-patient hospitalization within twenty-four (24) hours.  The hospitalization occurred on February 6, 2024, and was reported to OSHA on February 15, 2024.

29 CFR 1926.50(b): Provisions were not made prior to commencement of the project for prompt medical attention in case of serious injury:   a. At 5909 NW 43rd Ave., Gainesville, Florida, on or about February 6, 2024, the employer lacked an emergency response plan prior to commencement of the roofing work to ensure serious injuries that required immediate attention recieved prompt and appropriate medical treatment.    AB Construction, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.50(d)(1), which was contained in OSHA inspection number 1516625, citation number 2, item number 1, and was affirmed as a final order on June 17, 2021, with respect to a workplace located at 13 Green Winged Teal Rd, Fernandina Beach, Fl 32034.
