29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights were not bear on base plates and mud sills or other adequate firm foundation. a) On or about 12/13/2023, during the capital improvement project at the Wilson Academy School, the employer did not enforce the use of safe supports for scaffold poles. Employees were allowed to work on scaffolds without supporting the scaffold legs with mudsills to provide a firm foundation. Employees were exposed to falls and struck by hazards associated with scaffold collapse. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(4):Ladders were not used only for the purpose for which they were designed. a) On or about 12/13/2023, during the capital improvement project at the Wilson Academy School, the employer did not enforce safe ladder usage. Employees were allowed to work on step ladders without the use of the locking device or spreaders. Step ladders are not designed for use without the spreader or locking device in place. Employees were exposed to fall hazards due to the displacement of the step ladder. NO ABATEMENT CERTIFICATION REQUIRED
