29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Roof: Lot 10, Hidden Fields Subdivision, Hoschton, GA 30548: On or about 12/14/2023, the employer exposed employees to fall hazards of 28-feet from the eave of the 10/12 pitched roof, in that an adequate fall protection system was not used while using the roof as a walking/working surface. b) Roof: Lot 10, Hidden Fields Subdivision, Hoschton, GA 30548: On or about 12/14/2023, the employer exposed employees to fall hazards of 28-feet from the eave of the 10/12 pitched roof while performing roofing installation activities while connecting a personal fall protection system to a rope tied around the truss as an anchor.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations. a) Roof, Lot 10, Hidden Fields Subdivision, Hoschton, GA, Roof: On or about 12/14/2023, the employer exposed employees to struck-by hazards while performing hoisting operations of materials such as but not limited to shingle bundles and ridge caps with the TranzSporter (ModelTP250) material hoist. The system was changed to include the installation of a climbing rope and not the manufacturer's required wire rope.
