On or about December 14, 2023, the employer failed to fully document that a competent person inspected the rotary table of a Soilmec SR-75 Dedicated Drilling Rig to ensure it was in safe operating condition, was fully secured to the rig, and/or free from damage.

29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: a. On or about December 14, 2023, at the above addressed jobsite, employees involved in the drilling operation of deep foundation caissons, were exposed to the hazard of falling to the lower level of approximately fifty-eight (58) feet while working directly near or next to the two (2) foot six (6) inch unguarded caisson opening.
