OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by and crushing hazards during caisson drilling operations:  a)	On or about December 2, 2024, Stalworth Underground, LLC employees were exposed to struck-by, crushing, and/or caught-between hazards during caisson drilling operations of the CA 1-A shaft while walking and working inside of the Danger Zone of the Soilmec SR-75 CCS Drill Rig, while the machine was in operation.  b)	On or about December 2, 2024, Stalworth Underground, LLC employees were exposed to struck-by, crushing, and/or caught-between hazards during caisson drilling operations of the CA 1-A shaft while walking and working within the swing radius of the Soilmec SR-75 CCS Drill Rig, while the machine was in operation.  c)	On or about December 2, 2024, Stalworth Underground, LLC employees were exposed to struck-by and crushing hazards during caisson drilling operations of the CA 1-A shaft where attachments for a Soilmec SR-75 CCS Drill Rig where not securely stored in areas where employees were working.
