29 CFR  1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in �� 1926.502 (Appendix G to this subpart).  a) On or about 12/15/2023, the site located at Baillie Lumber, Railroad Street, Smyrna, NY 13464: Fall arrest systems did not conform to the criteria in 1926.502(d)(8) when the horizontal lifeline used by employees was not designed, installed and used under the supervision of a qualified person as part of a personal fall arrest system.  b) On or about 12/15/2023, the site located at Baillie Lumber, Railroad Street, Smyrna, NY 13464: Fall arrest systems did not conform to the criteria in 1926.502(d)(10)(i) when employees utilizing vertical lifelines were not attached to separate lifelines.  c) On or about 12/15/2023, the site located at Baillie Lumber, Railroad Street, Smyrna, NY 13464: Fall arrest systems did not conform to the criteria in 1926.502(d)(15)(i) when multiple employees connected to two vertical lifelines tied around a roof vent that was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  d) On or about 12/15/2023, the site located at Baillie Lumber, Railroad Street, Smyrna, NY 13464: Fall arrest systems did not conform to the criteria in 1926.502(d)(16)(iii) when employees were utilizing a fall arrest system which would allow for a free fall of 12 to 18 ft.

The employer shall institute a training program and ensure employee participation in the program.  a) On or about 12/15/2023, the site located at Baillie Lumber, Railroad Street, Smyrna, NY 13464: Employees installing metal roofing were not trained in the use and operation of the personal fall arrests systems and horizontal lifelines, the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems used on site, procedures to be followed to prevent falls to lower levels, through holes and/or openings and the requirements of this subpart. Employees were exposed to falls of 23 feet.
