29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) On December 11, 2023, and at times prior at 6790 US Highway 81 in Grafton, North Dakota, employees were exposed to amputation hazards while operating or working in close proximity to the Volmpack 12000 bagger (No. 99/205 Mach) that was not properly equipped with point of operation guarding.

This was not reported to OSHA until December 20, 2023.
