At a residential job site, located at 8783 Nature View Lane West, Jacksonville, FL 32217: On or about December 20, 2023, the employer exposed an employee to head injury hazards caused by falling or flying objects, in that head protection was not utilized by the employee, standing in the dumpster handing rolls of underlayment paper to other employees on the roof.

At a residential job site located at 8783 Nature Vie Lane West, Jacksonville, Florida 32217: On or about December 20, 2023, the employer exposed fiver employees to fall hazards ranging up a 24 feet fall hazard, in that fall protection was not utilized by employees while performing roofing activities. Blue Sky Florida Homes, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1660242, citation number 01, item number 002 and was affirmed as a final order on May 25, 2023, with respect to a workplace located at 4659 Post Street, Jacksonville, FL 32205.
