29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized, other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a.) Erie Strayer Company, Overhead door #5 area, on or about December 04, 2023, and times prior thereto: Employee removed the cover to the energized 480V control box for overhead door #5 to access the control lever while performing maintenance work. Personal protective equipment including, but not limited to, electrically rated gloves, arc rated long sleeve or coverall, arc rated pants, arc rated balaclava, and arc rated suit hood or face shield were not utilized while operating the lever in the door #5 control box.
