29 CFR 1910.178(o)(1):The employer did not ensure that only stable or safely arranged loads were handled: a) Custom Glass Solutions, Upper Sandusky, LLC, located at 12688 State Highway 67, Upper Sandusky, Ohio, 43351: On or about December 18, 2023, the employer failed to ensure the APEX hopper was handled in a manner that was stable or safely arranged on the forks of the Hyster #4850 (Model: 24-85S-21). As a result, top portion of the hopper displaced from the forks of the vehicle, exposing employees to struck-by hazards.
