29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Custom Glass Solutions, Upper Sandusky, LLC - Upper Sandusky, Ohio: On or about May 8, 2025, the employer did not develop, document, and utilize energy control procedures for authorized employees who maintain conveyor section 14 of the Autoloader. Employees were exposed to the rotating parts of the conveyor and pinch points created when a section of the conveyor raises. b. Custom Glass Solutions, Upper Sandusky, LLC - Upper Sandusky, Ohio: On or about May 8, 2025, the employer did not develop, document, and utilize energy control procedures for authorized employees who maintain conveyor section 15 of the Autoloader. Employees were exposed to the rotating parts of the conveyor. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a. Custom Glass Solutions, Upper Sandusky, LLC - Upper Sandusky, Ohio: On or about May 8, 2025, the employer did not ensure that employees who maintain equipment such as, but not limited to, the Autoloader conveyor sections, and robot were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees who performed preventative maintenance tasks were not trained in the applicable energy sources and methods and means necessary to isolate electrical and pneumatic energy sources. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a. Custom Glass Solutions, Upper Sandusky, LLC - Upper Sandusky, Ohio: On or about May 8, 2025, employees were exposed to injury from rotating parts and pinch points created by moving components of the conveyor, used to move glass in the tempering furnace area. The equipment was not locked out prior to performing inspection/preventive maintenance activities. The employer did not ensure that energy control application steps were implemented to control the equipment's electrical energy source prior to the servicing tasks - the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. b. Custom Glass Solutions, Upper Sandusky, LLC - Upper Sandusky, Ohio: On or about May 8, 2025, employes were exposed to injury from moving components of the Tempering Furnace Load Robot in that the equipment was not locked out prior to performing inspection/preventive maintenance activities.
