29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: Location: Non-ferrous Metal Room On or about December 20, 2023, a foot pedal controlling the power to an alligator shear was not adequately covered to prevent employees from accidentally stepping on the foot pedal, and activating the alligator shear's blade, exposing employees to lacerations and amputations.

Instance 2: On or about December 20, 2023, a foot pedal controlling the power to an alligator shear was not adequately covered to prevent employees from accidently stepping.
