ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Great Lake Metal Finishing, Inc. on or about 12/20/23 and times prior thereto: a flexible power cord to the Frigidaire Model #FFFC25M4TW was wedged between wood boards to keep the power cord off the ground.  The use of the flexible power cord was prohibited in that the cord was running along, and attached to, the building surfaces.
