The employer is failing to protect employee(s) from cave-in hazards. This was most recently documented on December 26, 2023, at 12112 S. 109th St. Papillion, Nebraska. The employer failed to ensure that employee(s) working in a trench were not exposed to cave-in hazards at depths greater than 11 feet.
