29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about December 27, 2023, at the facility located at 1201 D�cor Drive, Wausaukee, Wisconsin; the employer failed to ensure the guarding on Punch Press 13 (Denison HydroOILics, Model A, Serial No. 3022) was designed in a manner that prevented access to the hazard. The guard had a hinged section that easily allowed employees access to the danger zone of the point of operation.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. On or about February 6, 2024, at the facility located at 1201 D�cor Drive, Wausaukee, Wisconsin; the employer failed to ensure unused openings in boxes were effectively closed. Specifically, a box on Punch Press 10 (Havir Press Rite, Model Rite O, Serial No.: 3022) was not effectively closed, exposing employees to potential contact with electrical current.
