29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 27, 2024, employees engaged in maintenance repairs of the High Pressure Processing machinery at the Universal Pure facility at 1601 Pioneers Blvd, Lincoln, Nebraska activities, were exposed to struck by and amputation hazards in that the employer had not utilized energy isolation procedures to prevent the unexpected energization or startup of the machines, and/or the release of stored energy.
