This condition exposed employees to struck-by hazards associated with improper forklift operations.

This condition exposed employees to fall hazards and crushing hazards associated with the platform separating from the mast of the forklift.

Employees were exposed to various fall hazards from rough terrain forklifts, aerial lifts, the structure being framed, lack of pinning of rough terrain forklift elevated platforms, and eye injuries associated with power tool usage. This condition exposed employees to a variety of fall hazards identified on site.
