29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated (a)752 Main Street, Richfield, PA 17086 - Worksite - An employee was working from a JLG aerial boom lift without a lanyard attached to the basket exposing the employee to a fall of approximately 24 feet to the ground below, on or about October 31, 2023. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

(a)752 Main Street, Richfield, PA 17086 - Worksite - An employee was working from a portable extension ladder with a missing bottom rung and multiple damaged rungs at the top, exposing employees to fall hazards, on or about October 31, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
