29 CFR 1910.119(e)(3)(iv): The process hazard analysis did not address the consequences of failure of engineering and administrative controls: a) D3 Tank Farm, BVPV Styrenics LLC, on or about January 3, 2024, the Process Hazard Analysis Polystyrene D3 Unit Styropek - Beaver Valley Site did not address the consequences of failure of the engineering and administrative controls for a loss of nitrogen to the East/North Tank (T-3398) and West/South Day Tank (T-3393). The controls include a conservation vent (PSV-3517) sized for inbreathing, a pressure transmitter (PAL-3W80) with a low alarm to prompt operator response, and a level transmitter (LAL-3N18) interlocked to stop the tank pump (P-3397) for the East/North Tank and a conservation vent (PSV-3502), a pressure transmitter (PAL-3W78), and a level transmitter (LAL-3N16) interlocked to stop the tank pump (P-3395) for the West/South Day Tank. b) D3 Polystyrene Process Area, BVPV Styrenics LLC, on or about January 3, 2024, the Process Hazard Analysis Polystyrene D3 Unit Styropek - Beaver Valley Site did not address the consequences of failure of the rupture discs (PSE-3157/8) discharging to atmosphere, agitator low amp alarm (IAL-3179) prompting operator response and interlocked to initiate cooling, temperature control (TIC-3321) to allow full cooling water flow on reactor jacket, manual valve (HV-3207) to allow transfer of reactor contents to a dump box, and pressure transmitter (PAH-3154) with alarm to prompt operator response for loss of agitation on polystyrene reactors. c) D3 Polystyrene Process Area, BVPV Styrenics LLC, on or about January 3, 2024, the Process Hazard Analysis Polystyrene D3 Unit Styropek - Beaver Valley Site did not address the consequences of failure of the dual mechanical seal on reactor 301 agitator (A-3193) and area LEL monitors with alarm for a loss of nitrogen on reactor M.O. Pots. d) D3 Expandable Polystyrene Process Area, BVPV Styrenics LLC, on or about January 3, 2024, the Process Hazard Analysis Expanded Polystyrene D3/D4 Units Styropek - Beaver Valley Site did not address the consequences of failure of the double mechanical seal on reactor agitator, level switch with alarm (LAL-3Y87) on the M.O. Pot to prompt operator response, exhaust fans in reactor building, and area LEL monitors with audible and visual alarms for a loss of nitrogen on reactor M.O. Pots. e) D3 Expandable Polystyrene Process Area, BVPV Styrenics LLC, on or about January 3, 2024, the Process Hazard Analysis Expanded Polystyrene D3/D4 Units Styropek - Beaver Valley Site did not address the consequences of failure of the ZSC-3Y37 opening for purge sequence to start, and area LEL monitors with audible and visual alarm for a loss of nitrogen during reactor purge. Not addressing the consequences of failure of engineering and administrative controls can expose employees to fire hazards.
