29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) On or about January 2, 2024, and at times prior, employees were exposed to fall hazards where no fall protection was in place when working around floor holes. An employee suffered serious injuries after falling through a floor hole during cleaning operations in the area of the electric arc furnace.
