29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location: Throughout the residential construction site located at 8 Curtis Street, Somerville MA.  On or about 1/4/2024, several hazards existed on a residential construction site in areas readily accessible to employees on site. There was no competent person or supervisor assigned or available to provide frequent or regular safety inspections of the jobsite.  a) An employees was exposed to serious fall injury while working on a three story roof with no fall protection. b) Employees were exposed to injury from fall hazards from a scaffold located on the southern side of the home was not fully planked and cantilevered greater than 18 inches. c) Employees were exposed to injury from fall hazards from an extension ladder located in the rear side of the home was not placed at an appropriate angle. d) An employee was exposed to a struck by injury when manually hoisting construction materials from the ground to the third floor without wearing a hard hat. e) Employees were exposed to injury from a struck by hazards from a scaffold longer than 20 feet located on the northern side of the home which was left unsupported over more than half of its span.
