29 CFR  1910.147(c)(7)(iv):The employer did not certify that employee training had been accomplished and kept up to date:  a) Production Area: On or about January 4, 2024, and at times prior, the employer exposed employees to amputation hazards, in that the employer has not certified through names and dates that employees have been trained in lockout tagout when they are required to provide maintenance on the Trak Bed Mill Model DPMRX3, Hydmech Model H18A, and Messer Plasma Cutting Table Model TITAN.

29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Production Area: On or about January 4, 2024, and at times prior, the employer exposed employees to amputation hazards, in that machine guarding for the Trak Bed Mill Drill, Model DPMRX3, Serial # 255700 was not provided to employees while they drill single pin holes into metal bosses.  b) Production Area: On or about January 4, 2024, and at times prior, the employer exposed employees to amputation hazards, in that machine guarding for the Hydmech Horizontal Band Saw, Model H18A, Serial # B0620590T was not provided to employees while they cut raw metal material to make bosses.   c) Production Area: On or about January 4, 2024, and at times prior, the employer exposed employees to amputation hazards, in that machine guarding for the Messer Plasma Cutting Table, Model TITAN, Serial # TITAN8/8L-05-4415 was not provided to employees while they manufacture steel thumbs, rakes, stompers, and grapples.
