29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. a) In the aluminum welding area: On or about January 08, 2024, and at times prior thereto, the employer exposed employees to electrical shock hazards in that the electrical power cord connecting the Lincoln 256 welder to the 220-outlet had improper splices and connections, exposing the conductors while performing welding activities.

29 CFR 1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) In the aluminum building: On or about January 8, 2024, and at times prior thereto, the employer exposed employees to slip and trip hazards in that the walking-working surfaces in were kept in a cluttered condition with scraps of metal and other materials while employees were manufacturing trailers.
