29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about January 9, 2024, throughout the three buildings of the jobsite where employees performed hot work, multiple fire extinguishers were not mounted and/or identified. a) Fire Extinguisher #1: On the south side of the Fab Metal Shop, where acetylene tanks are stored, an extinguisher was not identified. b) Fire Extinguisher #2: On the north side of the Fab Metal Shop, by the exit door, an extinguisher was not identified. c) Fire Extinguisher #3: On the east side of the Fab Metal Shop, within the welding workstation, an extinguisher was not mounted or identified. d) Fire Extinguisher #4: On the east side of the Assembly Shop, an extinguisher was not identified. e) Fire Extinguisher #5: On the south side of the Machine Shop, where spray bottles are stored, an extinguisher was not mounted.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: a) On or about January 9, 2024, in the machine shop, employees, engaged in housekeeping operations, worked with a ZEP concentrated all purpose cleaner & degreaser (irritant) that was transferred into a secondary spray container without the secondary container being marked with an identifying label.
