29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about December 29, 2023 at a facility located at N3620 County Road D, in New London, WI; sanitation employees engaged in servicing and maintenance tasks on the Pre-Blend (Room 32) Diverter Conveyors' head pulleys were not provided training in the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control as an authorized employee.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the actions listed in and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 29, 2023, at a facility located at N3620 County Road D. in New London, WI; employees were exposed to machine hazards associated with the moving parts when they engaged in Sanitation activities on the Pre-Blend (Room 32) Diverter Conveyor 3 servicing the accumulation bin for Line 3. The employer did not ensure that employees implemented energy control application steps prior to allowing them to wash down the conveyors exposing them to ingoing nip points posed by the conveyor at its head pulley. The machine was not stopped or deenergized prior to the employee engaging in the servicing/maintenance activity. As a result, the remaining applicable energy control elements, involving machine preparation for shutdown [1910.147(d)(1)], machine or equipment shutdown [1910.147(d)(2)], isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
