29 CFR 1910.146(d)(1): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not implement the measures necessary to prevent unauthorized entry: On or about June 24, 2025 at a jobsite located at 4612 Speaker Rd. Kansas City, Kansas, employees were exposed to confined space hazards in that the employer did not ensure that employees did not have unauthorized access to permit required confined spaces. Employee broke the plane of the RTU 136 unit, a permit required confined space, resulting in an amputation.
