29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.) On or about  January, 10, 2024, at 1410 Malibu Circle, employees were exposed to a 9 feet 4 inches to 14 feet fall hazard, in that no fall protection was used while working on a 4:12 pitched roof removing and installing roof shingles.  b.) On or about January, 10, 2024, at 1420 Malibu Circle, employees were exposed to a 8 feet 7 inches fall hazard, in that no fall protection was used while working on a  4:12 pitched roof removing and installing roof shingles.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.) On or about  January 10, 2024, at 1410 Malibu Circle, employees were using an extension ladder to access the roof and the side rails were approximately 24 inches (2 rungs) past the roof landing , exposing employees to fall hazards of approximately 9 feet  4 inches.  b.) On or about January 10, 2024, at 1420 Malibu Circle employees were using an extension ladder to access the roof, and the side rails were approximately 18  inches (1.5 rungs) past the roof landing , exposing employees to fall hazards of approximately 8 feet 7 inches.
